Remember, the Green Sheet is not intended to be an analysis of the incident or a discussion of the factors behind the cause. It is supposed to be a quickly produced overview or summary of the events. It could be a learning document in the case where there are any obvious serious safety issues, like an equipment malfunction or a newly discovered hazard. They often include reminders and tips of the same basic safety guidelines we all hear everyday, WHETHER OR NOT those issues played a part in the injury/event. The Green Sheet is also a very incomplete document if you are looking for “mistakes” identified or “lessons” to be learned. You should really use (or wait for) the investigation document if you want any kind of factual study, conclusions or real causal factors. A lot of premature conclusions have happened a lot of times from people putting too much faith the in the Green Sheet’s initial attempt at an overview as a complete and final telling of the events.
It usually takes 18 to 24 months for the investigations to be completed, but all fire fighter fatalities in the country are investigated by NIOSH and can be found at the CDC website.
